VillageView Family Care Terms of Service

  1. ​Scope and Nature of Services:

    1. Service Offerings: VillageView Family Care provides primary care and urgent care services exclusively through telemedicine.

    2. Eligibility: Our services cater to adult patients aged 16 years and up.

    3. Service Areas: We currently offer our telemedicine services in Florida, Texas, and California.

 

  1. Payment and Billing:

    1. Pricing: Our self-pay rates are as follows:

      1. New Patients: $99.99

      2. Returning Patients: $99.99

    2. Insurance: We accept most commercial insurances and Medicare. However, we do not cater to marketplace plans or Medicaid plans. After your visit, claims are submitted to your insurance company. Depending on the specifics of your insurance plan, you may be responsible for additional payment.

    3. Payment Processing: Payments are processed securely through Stripe.

    4. No Show Policy: Patients who schedule appointments but fail to cancel within 48 hours or do not show up for their appointment will incur a fee:

      1. Return Patients: $20.00

      2. New Patients: $45.00

    5. SMS Communication Agreement

      1. SMS Communication Consent: By agreeing to these Terms of Service, you, as the patient, hereby grant VillageView Family Care the permission to send SMS messages to your designated mobile number for the purpose of health-related communications, appointment reminders, and other relevant updates.

      2. Opt-Out Provision: You have the right to opt-out of receiving SMS messages from VillageView Family Care at any time. To discontinue receiving SMS messages, you may follow the opt-out instructions provided in each SMS communication or contact (346) 299-1055 directly.

      3. Cost of SMS Messages: You acknowledge that, depending on your phone carrier contract, standard text messaging rates may apply to any SMS messages sent or received. VillageView Family Care is not responsible for any charges incurred by you from your mobile service provider as a result of receiving SMS messages from VillageView Family Care.

      4. Changes to Terms of Service: VillageView Family Care reserves the right to modify these SMS communication terms at any time. Any changes will be effective immediately upon posting the revised terms on www.villageviewfamilycare.com.

  1. User Responsibilities:

    1. Technology & Permissions: While users aren't required to create an account, they must grant necessary permissions to the doxy.me platform, the video software we utilize for consultations.

    2. Appointment Verification: Appointments are verified through email.

 

  1. Termination & Suspension:

    1. VillageView Family Care reserves the right to suspend or terminate any user's access to its services due to misuse, abuse, or violation of these terms.

 

  1. Dispute Resolution:

    1. In the event of a dispute arising between a user and VillageView Family Care, both parties agree first to try to resolve the matter informally. If the matter remains unresolved, it shall then be referred to binding arbitration in accordance with local jurisdiction.

 

  1. Limitations & Disclaimers:

    1. VillageView Family Care provides telemedicine consultations based on the information shared by patients. While we strive to offer the best medical advice, we cannot guarantee specific medical outcomes. Patients are advised to seek in-person medical care for emergencies or conditions beyond the scope of telemedicine.

    2. We do not bear responsibility for technical issues, including connectivity or equipment failures, that are outside our control.

 

  1. Governing Law:

    1. These terms of service are governed by the laws of the state in which the user receives the service.
